YOUTH2000 Group Registration

This Group Form must be filled out by the group leader.

After filling out this form please download the Individual Registration Form here. Every Chaperone and Participant needs a form sent into the Office of Youth Ministry. You can scan them and send in one email to robin.tomes@pastoral.org(Prefered) or mail to Robin Tomes at 600 Locust Street, Owensboro, KY 42301. Please remember that the USPS is very slow these days and if you are registering your group 2 weeks before Youth2000 it would be prudent to scan and email them or drop off the forms to the McRaith Catholic Center instead of mailing them.

****Please make sure all chaperones are in compliance with Diocesan Safe Environment policy before registering them as a chaperone. Please refer to Diocese of Owensboro Safe Environment Policy at https://owensborodiocese.org/safe-environment-policies/ for information on overnight trips, etc. There must always be 2 chaperones, even if you only have the 1 group of 7/10 ratio. (7 participants if middle and high school combined, 10 participants if High School only).

Registration is limited to the first 900 applicants.
