Catholic Schools Year-end

In the transitioning and updated certification guidelines, teachers no longer receive a certificate for each of the 15 courses they attend. Instead, each teacher is to document their own credits of formation each year. The year-end form is the accountability piece that insures all teachers are making progress toward Basic and then Advanced Certification. The new “Teacher Year-end Form” is below. The form is to be turned in to the principal and principals are to submit all teacher forms to the Office of Faith Formation by May 31 each year.
