Catholic Schools Year-end

In the transitioning and updated certification guidelines, teachers no longer receive a certificate for each of the 15 courses they attend. Instead, each teacher is to document their own hours of formation each year. The year-end form is the accountability piece that insures all teachers are making progress toward Basic and then Advanced Certification. The new “Catholic Schools Year-end Form” is below. The form is due to the Office of Faith Formation by May 10 each year.
